چکیده

DURING the repair of congenital defects of the venitricular septum, various disturbances of cardiac function may occur.1'Permanent complete heart block has been the most serious of these disturbances. Right bundle-branch block seems benign. Inereased knowledge of the anatomic features of the atrioventricular (AV) conduction system has resulted in surgical technics that have nearly eliminated complete heart block.5 These teehnies are based on the assumption that trauma to the AV conduction system occurring during surgical repair of a ventricular septal defect is the cause of postoperative functional disturbances. However, studies documenting an anatomic cause for heart block are few; Reemtsma and associates6 investigated the lesions responsible for fatal postoperative complete heart block in three cases-one of isolated ventricular septal defect, one of tetralogy of Fallot, and one of common AV canal. There is general agreement that permanent complete heart block is almost always associated with a lesion in the proximal portion of the AV conduction system.7' 8 There is not such general agreement concerning the morphology of the conduction system of hearts with right bundle-branch block.7 The present study of cases operated upon during a period when heart block was not

Intraoperative recording of specialized atrioventricular conduction tissue electrograms in 47 patients.

Intraoperative mapping of the specialized atrioventricular conduction system was performed in 47 patients during cardiac surgery. Specialized conduction tissue electrograms were identified in 37, and atrioventricular conduction preserved in 92%. Specialized conduction tissue was identified in 27 patients with atrioventricular canal defect: complete heart block was avoided in 25. Conduction tiss...

Modulation of extracellular atrioventricular node field potential pattern and ventricular rhythm by morphine in experimental atrial fibrillation in isolated rabbit heart

Introduction: Endorphins are produced by cardiomyocytes, and exert different effects on the heart. The aim of the present study is to assess morphine effects on extracellular atrioventricular (AV) node field potential pattern and ventricular rhythm of isolated rabbit heart during experimental atrial fibrillation (AF). Methods: Effects of different concentrations of morphine (10, 20, 50 and 1...

Anatomic effects of cryoablation of the atrioventricular conduction system.

Because of the value of cryoablation of the atrioventricular (AV) conduction system in treatment of refractory cardiac rhythm disorders, the anatomic effects of cryoablation on the cardiac conduction system must be defined. In this report we summarize studies done on four patients who had intractable recurrent supraventricular tachyarrhythmias or or refractory atrial flutter-fibrillation. They ...

are explained on the basis of anatomic lesions in specific sites of the conduction system or its blood supply. Second-degree A-V block of Mobitz type II, which may be a precursor of complete (third-degree) heart block, commonly is associated with fibrotic lesions of uncertain etiology in the branching part of the bundle of His or the bundle branches. Ischemic lesions are found less often, and o...
